Assessing a Client’s Temperature

  • Tools:
    • Digital forehead thermometer
  • Steps:
    1. Seat client comfortably
    2. Take thermometer and place it facing the client’s forehead
    3. Hold in place and pull the trigger to scan the client’s temperature
    4. When the thermometer is finished measuring, document the client’s temperature as _____ degrees (°F)
  • Temperature can also be retrieved:
    • Orally
    • Rectally
    • Axillary
    • Tympanic (Ear)
  • Normal Values:
    • Average normal body temperature: 98.6°F
    • Normal range between 97°F to 99°F
    • Temperature above 100.4°F indicates a fever
